Anemia as very late-onset cytomegalovirus disease after kidney transplantation.

چکیده
Very late-onset cytomegalovirus (CMV) disease after solid organ transplantation is not associated with classic risk factors; therefore, it does not follow a pattern of predictable appearance. A high index of suspicion is necessary to make an accurate diagnosis. Anemia has multiple etiologies among kidney transplanted recipients, and CMV could be one of them. We report the case of a kidney recipient with anemia refractory to treatment which proved to be secondary to extremely late CMV digestive disease.

Cytomegalovirus Infection Associated with Acute Onset of Inflammatory Bowel Disease Forty Years after Kidney Transplantation: A Case Report
We observed a patient with de novo acute Crohn’s disease in the setting of very late cytomegalovirus activation in the blood and in the colon, 40 years after kidney transplantation, which has not been reported to date. Antiviral therapy associated with everolimus was effective without major adverse events. It remains unclear if cytomegalovirus infection was the trigger or the consequence of acu...
متن کاملImpact of cytomegalovirus disease in D+/R- kidney transplant patients receiving 6 months low-dose valganciclovir prophylaxis.
Late-onset cytomegalovirus (CMV) disease remains common in CMV serology naïve kidney transplant patients of CMV serology positive organs (D+/R-) despite the use of antiviral prophylaxis. We studied clinical efficacy of 6-month low-dose valganciclovir (VGCV) prophylaxis, risk factors for late-onset CMV disease and its impact on kidney transplant outcomes. Between October 2005 and December 2009, ...
متن کاملIncidence and risk factors for cytomegalovirus in kidney transplant patients in Babol, northern Iran
Background: Cytomegalovirus (CMV) disease is an important cause of death and possibly transplant rejection in kidney transplant (KT) patients. This study was conducted to investigate the incidence and risk factors of CMV disease in kidney transplant patients. Methods: All end-stage renal disease (ESRD) patients who underwent kidney transplantation during 1998-2014 and their donors were assessed...
متن کاملSimultaneous cytomegalovirus and Mycobacterium tuberculosis infection presenting as hemorrhagic colitis 3 years after a kidney transplant.
Cytomegalovirus is an important cause of colonic disease in solid organ transplant recipients. Although several reports have shown that simultaneous infection with other pathogens may aggravate the course of cytomegalovirus colitis, to our knowledge, no case of colitis caused by simultaneous cytomegalovirus and Mycobacterium tuberculosis has been previously reported. We describe a case of hemor...
متن کاملLate-onset cytomegalovirus-associated interstitial nephritis in a kidney transplant.
Cytomegalovirus is the most important viral infection in kidney transplants, but rarely affects the allograft after the sixth month posttransplantation. We present a patient who developed renal failure eighteen months posttransplant; a kidney biopsy showed cytomegalovirus inclusions, acute tubular necrosis and mild interstitial nephritis. After intravenous ganciclovir, renal function transientl...
